2011 Special Gift Grant Program
The Rest Haven Children’s Health Fund (Rest Haven) Board of Directors is pleased to announce its
Special Gift Grant Program for 2011.
Rest Haven is a California 501c (3) nonprofit corporation dedicated to addressing the health needs of children. Rest Haven’s
mission is to provide funds for health related services for children in San Diego and Imperial counties when no other funds are available.
Rest Haven will make available up to
$200,000 in grants during 2011. Rest Haven’s focus this year is to assist the communities in San Diego and Imperial Counties to
increase their capacity to provide health care to meet the physical, medical and/or emotional needs of children, ages 0-18 years. Priority will be given to those requests that demonstrate an identified community need, the ability to increase their capacity to meet that need, outreach to underserved children and the sustainability of the project. Community Clinics/Health Centers are strongly encouraged to submit proposals that are consistent with the above criteria. The Rest Haven Board will give strong consideration to capital outlay requests that meet the above criteria.
These
one-time grants will be available on a competitive basis with funding of projects/programs being awarded in July for expenditure commencing
September 1, 2011. Grant awards can be for up to one year.
All awardees are expected to provide a mid-year
report by
June 15, 2012, followed by a final report to the Rest Haven Board of Directors detailing the manner in which the grant monies have been used and documenting results and progress. Should an awardee not submit a final report, future applications for funding will not be considered.
DEADLINE FOR SUBMISSION OF GRANT REQUESTS
The deadline for submission for a 2011 grant request is May 6, 2011 for awards. The grant request must be postmarked by this date in order to be considered. All grant requests will be reviewed through a board committee process and approved by the full Board of Directors. All applicants will be notified in writing prior to September 1, 2011. Rest Haven reserves the right to reject any request submitted for any reason without indicating reasons for said rejection.
FORMAT AND FILING OF PROPOSALS
All requests must be typed with each section of the request tabbed and labeled in the order listed below:

Include the following: name of the agency/organization; dollar amount of the request; the name and title of the principal contact along with their telephone and fax numbers & e-mail address; agency’s website if applicable; project title; applicant’s address
- Table of Contents
The table of contents shall be complete and clear, listing headings and pages as follows:
- Cover Letter
- Background and Description of Agency
- Statement of Project Relevance to Rest Haven’s Mission
- Specific Request - Outcomes and Goals and Details of Expenditures
- Timelines for Completion
- Budget and Justification
- Cover Letter
The cover letter shall be brief and shall include a description of the agency requesting the grant along with the title of the project and how it fits into Rest Haven’s mission. The letter must be signed by the person with legal authority to bind the agency.

Description of agency along with agency’s purpose and population served
- Statement of Project Relevance
Description of need and how the project will address the need
- Specific Request - Outcomes and Goals and Details of Expenditures
Project goals and objectives and criteria used to measure outcomes. What and how grant monies will be expended to meet the goals of the project
- Timelines for Completion
Specific timelines for meeting and completion of objectives
- Cover Page
Specific dates grant monies are being requested for and project’s budget including overhead. No requests will be accepted with more than 4% administrative overhead.
- Sixteen copies of the request must be submitted.
- Proposals should not exceed 16 pages including attachments.
